Soccer Participation Disclaimer * I, _____ , give permission to the registered camper to participate in all the activities of the Global Soccer Academy and submit he/she is physically fit to participate in athletic activities. I/We understand that there are certain risks of injury in the participation of this sport and I am willing to assume these risks. I/We authorize the staff to administer first aid in case of emergency. I/We waive and release any and all rights and claims for damages, and hold harmless the staff and instructors for any and all responsibilities for injuries or illness.
The Global Soccer Academy also uses pictures and video taken during the camp for educational and promotional purposes and I/We allow the use of these for those purposes.
